Medical Records

Your medical records are required for a Medical Marijuana certificate. Florida requires Medical Cannabis certifications to be authorized after medical history review, exam and lengthy documentation done by our staff. Medical records are required (example: last office visit note, print-out from patient portal, proof of disability) for verification that you have a lasting diagnosis.

We can assist with obtaining such documents with a Records Request Form.

To help, you can do one of the following:

1. Digital Request. Request we email a digital signature Records Request form

2. PDF. Request we email a PDF version of the Records Request form for you to complete and provide to your doctor

or

3. Paper. Stop by the clinic to fill out the Records Request form

Scheduling

 Once scheduled, an Acuity Appointment Confirmation email will be sent to you with further instructions and a medical history survey.  

a. Upload medical records and Driver’s License 

b. Fill out the Medical History Forms 

c. Fill out Contact/Demographics information. 

All information requested is necessary for our process or legally required for the Department of Health Registry.  It is more helpful for all to have done in advance.

Education

Watch Dr. McCain’s Educational Videos on our  YouTube Channel  for lessons about the legal process, the Science behind Medicinal Cannabis, and Recommendations for use. Patient’s generally appreciate watching these prior to the appointment so the follow-up questions and discussion can be more detailed.

ID Card and Application

The Florida Department of Health manages the applications for the ID card. They require that you complete their application and assess a $75 annual fee directly to the patient. If you need assistance with completing the state application, our staff can guide you through the process after your initial visit.
